the online system will only allow you to include 1 page of the statement or so...so you'll have to submit a copy of your statement (20 pages or whatever) by mail as well, however you'll get your case number much faster if you'll apply online.

at the end of the online application when your case number we'll be generated, you'll see an outline of what you'll need to submit by mail in addition to your online application and the statement should be one of those things.

paper clips will work better

I think that presentation matters, but if the case is weak it won't help

Ya zapolnila on-line application for J-1 waiver on Department of State Website. Sladkaya, spasibo za poyasneniya. On-line application proyasnyaet mnogie momenti. Posle togo kak vse formi zapolneni, visvechivaetsya check list s dokumentami kotorie neobhodimi dlya otpravki.

Also you get a phf file with form DS-3035 filled out and a page to be sent to INS (in the case of a waiver based on fear of persecution).

The package also generates one page of the statement of reason. Should I include this page or just include the full statement in a different form. I still will have to include the full statement because it got cut off and footnotes were not preserved. Do I need a one page version of it as well?

You should include all pages of your statement with your printed application (I don't think that you'll need one-page that you've printed), I am sure that they'll be considering whatever you'll send them by mail, it's unusual to have only one-page statement (esp. for your type of waiver), so I think they'll understand
you'll be able to check your status online once they'll process it in a week or so, if you won't see "Statement of Reason--received" online, you should probably send it again

USCIS requires submitting originals of all documents. Should I really send the original of IAP-66? It clearly says that one needs to submit I-94 (original) with I-612, but if I choose to send some documents, do I really need to send originals? What if they loose them?
